Why Your Lab Tests Are

Why Your Lab Tests Are "Normal" But You Still Feel Awful

A doctor’s guide to uncovering root causes beyond standard TSH testing.

Your thyroid gland produces some of the most important hormones in your entire body.

But what happens if you have your thyroid removed? Or destroyed?

Are you able to survive?

The answer is yes and it has to do with the fact that thyroid medication contains the same hormones that your body produces naturally.

The only problem is that once your thyroid is removed or destroyed, you will be required to take thyroid medication for the rest of your life.

In addition, it can be difficult to optimize your medication to try and mimic what your thyroid gland would produce naturally.

In this article, we are going to discuss the importance of thyroid hormones, what happens if you have your thyroid removed (or destroyed), the side effects you may experience, and how to optimize your medication afterward.

Your step-by-step plan to reduce fatigue, hair loss & stubborn weight.

Doctor Summary (TL;DR)

Standard TSH tests often miss functional hypothyroidism; addressing conversion and absorption is key to symptom relief.

  • T4 to T3 conversion issues are common but rarely tested.
  • Nutrient deficiencies like selenium and zinc can mimic thyroid symptoms.
  • Optimal gut health is crucial for thyroid hormone function.

Common Mistake Box ⚠️: Treating symptoms individually instead of addressing the root cause.

Why Your "Normal" Labs Are Misleading

TSH only tells part of the story.

Doctor Note I’ve seen hundreds of patients with "normal" TSH who feel significantly better once we optimize their full thyroid panel.

The T4 to T3 Bottleneck

Most standard treatments focus on T4 (Levothyroxine). Think of T4 as "storage" energy. It sits in your system waiting to be used. But your body runs on T3 (active energy).

If your body is under stress, inflamed, or lacking specific nutrients, it cannot convert T4 into T3. Instead, it converts it into "Reverse T3," which actually blocks your energy receptors. This is like pressing the gas and brake at the same time.

Common mistake ⚠️: Treating symptoms individually instead of addressing the root cause.

Doctor Protocol Step 1 of 3

Fix absorption first

Most symptomatic patients skip this step.

  • Optimize stomach acid for nutrient breakdown.
  • Repair leaky gut to reduce inflammation.
  • Ensure adequate intake of key co-nutrients.

CLINICAL TABLE

Symptom to Root Cause Clinical Map

Symptom Potential Root Cause What to test (labs)
Chronic Fatigue Poor T4 to T3 Conversion Free T3, Reverse T3, Iron Panel
Hair Loss Nutrient Deficiencies / Autoimmunity Ferritin, Zinc, Vitamin D, TPO Antibodies
Stubborn Weight Insulin Resistance / Low Metabolism Fasting Insulin, HbA1c, Free T3

These are the most common patterns I see clinically.
